Who are Flexible Home Support Services for?
Flexible Home Support Services is a post-hospital discharge program for people who have completed the My Aged Care assessment process, been approved for a Home Care Package and are still on the waiting list.
The aim of the program is to provide short-term home support for those who are at significant risk – for example, those who are unable to access support through the Commonwealth Home Support Programme and face readmission to hospital or homelessness.
Referrals for Flexible Home Support Services are made by the Aged Care Assessment Team (ACAT).
How long does the service last?
All programs under this service provide a minimum of 6 weeks’ of care, with individual timeframes dependent on each person’s circumstance. The program ends when your Home Care Package is allocated and services are put in place.
How much does the service cost?
Flexible Home Support Services is funded by the Western NSW Local Health District. There are no customer contributions for your care.
What is included in the service?
To help keep you safe and well at home, the program provides services that are equivalent to your approved Home Care Package level of care.
