If you are being discharged from a NSW hospital, Out of Hospital Care can help you transition safely back home and regain your independence.

Getting you back on your feet

We'll support you in different ways once you leave hospital, so you can re-establish daily living skills, maximise your independence and continue to engage in life.

Case management

An experienced LiveBetter case manager will coordinate your services and ensure they meet your individual needs.

Essential services

We'll give you a hand with tasks like showering, transferring in and out of bed, shopping and meal preparation.

Community support

We're here if you need help, whether it's domestic assistance, transport to appointments or a check in phone call.

How does Out of Hospital Care work?

Each of these programs provides 6 weeks of low level home care services, covering things like showering, shopping, transport and meal preparation. The aim is to encourage an early and safe discharge from hospital, and prevent avoidable readmission.

LiveBetter provides Out of Hospital Care to people living in the Western NSW Local Health District. To find out if you are in this region, please see the district map.

Who is eligible for Out of Hospital Care?

LiveBetter’s Out of Hospital Care services is for people living in the Western NSW Local Health District.

Each of the 3 programs – ComPacks, SASH and End of Life care– have different eligibility criteria. Eligibility is determined by NSW Health and screened by the Aged Care Access Centre.

How much does Out of Hospital Care cost?

Out of Hospital Care programs are subsidised by NSW Health. For ComPacks and SASH, you will be asked to make a contribution of $10 a week, with total contributions capped at $60. This amount is negotiable, depending on your financial circumstances.

The End of Life care program doesn’t have a customer contribution.

How long does Out of Hospital Care last?

Out of Hospital Care programs provide 6 weeks of care for patients being discharged from hospital. In some cases, the discharge planner may recommend a Transitional Aged Care Package to meet your needs.

Out of Hospital Care with LiveBetter

LiveBetter’s strong partnership with the Western NSW Local Health District enables us to provide continuity of care from hospital to home. Our team responds quickly and communicates well with customers, giving you confidence that you’ll be well supported once you return home.

We also encourage our customers to have choice over who provides their care. While all of our programs are coordinated by a LiveBetter case manager, you can choose to have your care services provided by our team, or through external providers.
