If you’re about to be discharged from a public hospital in NSW and think you could do with a hand while you get back on your feet, then the Out of Hospital Care (OHC) Program may be able to provide assistance and support for a smooth and safe transition home.
The Out of Hospital Care Program is funded by the NSW Ministry of Health and can provide you with a range of services for up to six weeks in the comfort of your home.
The NSW Health OHC Program includes the following packages:
Community Packages (ComPacks)
Safe and Supported at Home (SASH)
End of Life (EoL)
These packages support hospital discharges and prevent avoidable admissions by providing short to medium-term non-clinical case management and services to eligible patients and their families/carers.
A Case Manager will develop a care plan with you and arrange services to be delivered in your home.
Services may include things like:
Personal care: assistance with bathing, oral care, dressing, hygiene, dressing, grooming and monitoring medication
Domestic Assistance: house cleaning, vacuuming, mopping, laundry and shopping
Meals: meals delivered to your home or meal preparation
Transport: for medical and other appointments
Social Support: accompaniment to appointments, shopping and paying bills
Respite: In home respite to support carers.
OHC is available to people who are either inpatients of a NSW Health public hospital or have been referred for OHC services by a NSW Health community team member. Each OHC program has specific eligibility requirements.
You will be referred to our Out of Hospital Care team by your hospital clinician.
You will be asked to contribute up to $10 per week towards your ComPacks/SASH package. There is no charge for the End of Life program.
We can also link you to other community care services that suit your needs, making your transition home safer, smoother and more secure.
