Our Out of Hospital Care Services
How does Out of Hospital Care work?
Each of these programs provides short-term low level home care services, covering things like showering, shopping, transport and meal preparation. The aim is to encourage an early and safe discharge from hospital, and prevent avoidable readmission.
LiveBetter provides out of hospital services to people living in the Western NSW, Hunter New England and Far West Local Health Districts. To find out if you are in one of these regions, please visit the NSW Health Government website.
Who is eligible for Out of Hospital Care?
LiveBetter’s Out of Hospital Care services is for people living in the Western NSW, Hunter New England and Far West Local Health Districts.
Each of the programs have different eligibility criteria. Eligibility is determined by NSW Health and screened by the Aged Care Access Centre.
How long does Out of Hospital Care last?
Out of Hospital Care programs provide between 6-12 weeks of care for patients being discharged from hospital.
Out of Hospital Care with LiveBetter
LiveBetter’s strong partnership with NSW Health enables us to provide continuity of care from hospital to home. Our team responds quickly and communicates well with customers, giving you confidence that you’ll be well supported once you return home.
We also encourage our customers to have choice over who provides their care. While all of our programs are coordinated by a LiveBetter case manager, you can choose to have your care services provided by our team, or through external providers.
