The Assistive Technology and Home Modifications (AT-HM) Scheme explained

With Support at Home just around the corner, we've taken a closer look at the Assistive Technology and Home Modifications (AT-HM) Scheme – what it is and what it means for you.
8th July 2025

The launch of Support at Home will bring changes and improvements to the way home care services are provided. We want to make sure you have all the information you need to understand these changes and make the most of these improvements.

This month, we’re looking at the Assistive Technology and Home Modifications (AT-HM) Scheme – what it is and what it means for you.

The Assistive Technology and Home Modifications (AT-HM) Scheme

Assistive technology and home modifications play a key role in enhancing independence and improving safety, offering older Australians tailored solutions that ensure a better quality of life.

With the new Assistive Technology and Home Modifications (AT-HM) Scheme, you’ll be able to access the assistive technology and home modifications you need, without tapping into your quarterly budget for ongoing services.

Your assessor can approve you for an assistive technology funding tier, a home modifications funding tier, or both, at the point of assessment. This means you can rest assured knowing you’ll have the support you need to live comfortably, safely, and independently in your own home.

What is Assistive Technology?

Assistive technology is the term used to describe the devices or equipment that help you live safely and independently at home and in your community. They help you with tasks you may struggle to complete on your own and can make a huge difference in your daily life.

Some examples of Assistive Technology:

  • Walking equipment like walking sticks, walking frames, or wheelchairs
  • Toileting supports, such as bedpans and commodes
  • Bathing devices like shower chairs and bath boards

Assistive Technology Funding Tiers*

The scheme offers different funding levels to suit your needs:

  • Low: Under $500
  • Medium: Up to $2,000
  • High: Up to $15,000

*These funding amounts will be finalised before 1 November 2025.

Additional Information:

  • If you need an item that costs more than $15,000, you’ll still be able to access it, but you will require a valid prescription from an allied health professional.
  • The funding covers not only the equipment but also any prescriptions from allied health professionals and additional wrap-around support to ensure you use the items safely and effectively.

What are Home Modifications?

Home modifications are the changes you make to your home to ensure it’s safer and more accessible. They enable you to live in your own home with greater ease and independence.

Some examples of Home Modifications:

  • Grab rails in the shower or bathroom
  • Handrails inside and outside
  • Ramps or stair lifts
  • Non-slip mats for floors and stairs

In some cases, more significant changes can be approved, such as:

  • Redesigning bathrooms for better accessibility
  • Widening doorways or passages for wheelchair access

Home Modifications Funding Tiers*

The funding for home modifications follows the same structure as assistive technology:

  • Low: Under $500
  • Medium: Up to $2,000
  • High: Up to $15,000

*These funding amounts will be finalised before 1 July 2025.

The AT-HM List

The AT-HM List includes all the approved products and modifications that you can access through the scheme. This list will be reviewed annually to make sure it includes the latest and best options available.

The list organises items based on how they should be used:

  1. Low risk: Simple, everyday items that don’t need a prescription or customisation.
  2. Under advice: Items that are generally safe but could benefit from professional advice for proper installation or use.
  3. Prescribed: More complex items that require a prescription* from a health professional.

*Further information on who may prescribe assistive technology and home modifications under the AT-HM Scheme in the coming months

What’s Not Covered by the AT-HM Scheme?

There are a few items that are not covered by the AT-HM Scheme, including:

  • Products that are funded by other government services
  • Regular household items or discretionary expenses (e.g., televisions or general furniture)
  • Items used at work or for mass transit
  • Products for children

Please note that there may be some items that are only available through the AT-HM Scheme under specific circumstances.

National Assistive Technology Loans Scheme

Another innovative addition to the AT–HM Scheme is the National Assistive Technology Loans Scheme (AT Loans Scheme). This scheme, which is currently being trialled, will allow you to borrow assistive technology to assess whether it is suitable before deciding whether to buy it.

Here’s how it will work:

  • If you’re prescribed an item from the AT-HM List, your prescriber will check if it’s available to borrow through the AT Loans Scheme in your area.
  • If the item isn’t available for loan, we’ll support you in purchasing it with the AT-HM funds.

This scheme will make it easier and more affordable to access the technology you need, with the flexibility to change equipment as your needs evolve.
