How Complex Clinical Care works
Our services are totally unique and personalised to your individual circumstances.
It’s also a team effort. We’ll partner with you, your family and your GP, and we can also train your carers, so they’re aware of your specific needs. Our Nurses are available for both short-term and long-term clinical supports.

Support designed just for you
Everyone’s needs and goals are different. Your need for support may begin with your GP’s diagnosis, or a more complex process, for example through a Support Coordinator or other referral pathways.
Whether you need a one-off consultation or lifelong care, or something in between, we make sure to fully understand your circumstances so that your care is completely personalised and holistic.
You’ll be supported by qualified clinicians who are dedicated to making sure you or your loved one is stable and functioning as well as able to maintain or improve quality of life.
And to make sure you’re in familiar and comfortable surroundings, we provide Complex Clinical Care services in your own home and community.
